The self screening test below and the associated informational pages do not provide a diagnosis of drug abuse, drug dependence, or any other coexisting medical condition. The information provided here cannot substitute for a full evaluation by a certified substance abuse counselor, doctor, or drug rehab facility. It should only be used as a guide to understanding your alcohol and drug use. It is provided for harm reduction and educational purposes only.

Step 1: Determine if You have an Addiction with a Self Screening Test

Screening for alcohol and other drug abuse can be used to initiate the process of assessment by identifying your possible problems and determining whether you may have an addiction.

Self Administered Alcohol and Drug Addiction Screening Test (pdf) - used under the public domain from SAMHSA TIP 11. You can print the form and fill it out or use a piece of paper to write in your answers for scoring. The scoring directions appear below the screening form. You score the form yourself and only you see the answers. If you scored a 3 or below, it is most likely that you don't have an addiction. If you scored a 4 or greater you are at higher risk and you may have an addiction, so move on to Step 2.
